  Talent Show Design Contest Entry Form The logo for this year's Talent Show will be designed by YOU! The show's theme is April Fool's. Contest entries mus be in black & white (or pencil) and fit within the rectangle on the entry form. All entries must be in the collection box int he main lobby by Monday, December 19. Good luck!
November Willard Wear Order Form Order Willard Wear for the Holidays. New products are available. Orders must be received no later than Thursday, December 1st to ensure Holiday delivery.
  Boxtops Contest Entry Form A Thanksgiving themed entry form for the Boxtops contest. Submit your Boxtops now!
October Boxtops Contest Entry Form Entry form for the Boxtops contest that runs until December 2011. Print out and attach Boxtops. Or simple send the Boxtops in an an envelope of baggie. In either case, please include your child's teacher's name so that the class can be credited for the contest.

Bus Contract
Bus Contract Cover Letter

 In order to promote safe bus behavior, we are sending home a Bus Agreement for all students to review with their parents. We are asking all students and parents to sign this agreement and return it to the classroom teacher by September 30.

 

 PTG 2010-2011 Membership Form Willard PTG Membership application for the 2010-2011 school year. PTG Members receive a copy of the Student Directory. Additional copies are available for $5 each. These are the 2010-2011 forms now, but can be used for the new school year until the new forms are available electronically.
  Directory Update Form Form for updating information to be included in the 2010-2011 Willard School Directory. These are the 2010-2011 forms now, but can be used for the new school year until the new forms are available electronically.
  Cell Phone Information Form The Willard PTG offers the option of adding cell phone numbers to the Willard School Directory. If you are interested in including your cell phone number, you must complete this form and return it to the main office by Friday, September, 16, 2011.
